I am having difficulty completing the survey online - can I just fax my responses?
We cannot accept fax updates — all of the submissions must go through the web survey so all of the metrics are validated through the online platform. Once you submit a page (section) and arrive at the next section, your data is stored in the database. If you do not get to the next section, then no updates are stored — this is to ensure data integrity with our on-page validation programming.
